This is an independent contractor role.
Responsibilities:
Patient Care:
- Assess the health status and needs of elderly residents upon admission and regularly thereafter.
- Develop and implement individualized care plans based on comprehensive assessments and resident preferences.
- Administer medications, treatments, and procedures in accordance with physician orders and established protocols.
- Monitor vital signs, symptoms, and changes in condition, and take appropriate action as necessary.
- Provide assistance with activities of daily living, mobility, and personal hygiene as needed.
- Respond promptly to medical emergencies and provide appropriate interventions.
- Foster a compassionate and supportive environment for residents and their families.
- Collaborate with physicians, therapists, dieticians and other healthcare professionals to coordinate resident care and services.
- Communicate effectively with residents, families, and staff to ensure understanding of care plans, treatment options, and progress.
- Participate in interdisciplinary team meetings and contribute insights and recommendations for resident care and quality improvement initiatives.
- Document all assessments, interventions, and communications accurately and comprehensively in residents' medical records.
- Extensive weekly, monthly and quarterly written and verbal reporting.
- Written newsletters to residents, hosting talks and undertaking healthcare events.
- Responsible for managing 6 staff members including 5 carers and 1 supervisor.
- Provide leadership, guidance, and support to caregivers.
- Delegate tasks appropriately and ensure that all team members adhere to established policies, procedures, and regulatory requirements.
- Conduct training sessions and educational programs to enhance staff knowledge and skills in geriatric care, safety protocols, and resident communication.
- Ensure compliance with all relevant regulatory standards, including state licensing requirements, infection control guidelines, and resident rights.
- Conduct regular audits and assessments to evaluate the quality of nursing care and identify areas for improvement.
- Implement corrective actions and follow-up measures as needed to address deficiencies and enhance resident outcomes.
- Registered Nurse (RN) license in good standing.
- Bachelor's degree in nursing or related field (preferred).
- Proven experience in geriatric nursing or long-term care settings.
- Strong clinical assessment and critical thinking skills.
- Proficient in Microsoft (Word, Excel, Outlook and PowerPoint)
- Excellent communication and interpersonal abilities.
- Leadership qualities with a focus on teamwork and collaboration.
- Proficiency in electronic health records and computerized systems.
- Commitment to upholding ethical standards and promoting resident dignity and autonomy.
