Therapeutic management of long-term conditions including Cardiovascular disease, Stroke prevention in Atrial Fibrillation (AF), Osteoporosis, Respiratory, Diabetes, Women’s health and chronic headaches. This included conducting therapy reviews and patient clinics in medical practices with the aim of enhancing patient care, optimising treatment and reducing hospital admissions. ¥ Delivered clinical therapy reviews in over 300 medical practices ensuring that patients are receiving the best possible care in line with national guidelines. ¥ Headed a Clinical Commissioning Group (CCG)- wide Stroke Prevention in AF project resulting in projected 30% reduction in AF-related strokes and 10% increase in anticoagulation by delivering therapy reviews and patient clinics in over 100 medical surgeries across the Sandwell and West Birmingham CCG. ¥ Redesigned the anticoagulation treatment algorithm for Sandwell and West Birmingham medical practices that bridged the interface between primary and secondary care. ¥ Overhauled and implemented a repeat prescription processing framework for administrative staff within a busy partnership of medical surgeries. ¥ Contributed to regional sales achievement of Quality Outcomes Framework (QOF) Enhancement service which assists medical practices by enhancing their chronic disease prevalence and maximising QOF income. ¥ Led and implemented a regional Electroceutical review service worth £120k in four medical practices that evaluated the impact of vagus nerve stimulation in patients suffering with chronic headaches, anxiety and depression resulting in a reduction in secondary care referrals, medication usage and doctor appointments. ¥ Realised ~£25k prescribing savings across a network of 4 Leicestershire medical surgeries through cost-effective prescribing analysis within the NHS Quality, Innovation, Productivity and Prevention (QIPP) initiative ensuring that prescribed drugs comply with latest NICE (National Institute of Clinical Excellence) guidelines. ¥ Collaborated with Research in Real Life by recruiting and monitoring patients for respiratory clinical trials through conducting spirometry along with outcomes assessments for the duration of the trial. ¥ Conducted medication reviews for Nursing Home patients ensuring appropriate usage of medication thereby reducing drug burden and medication wastage. ¥ Induction and coaching of over 20 new pharmacists and instilling company ethos and values.
