The shift pattern is Monday-Sunday, 5 days over 7.

An exciting opportunity has arisen for a Care Home Liaison Nurse (CHLN) to join our multi-disciplinary Care Home Support Team in East Dunbartonshire. The post holder will use a collaborative and co-ordinated approach to provide enhanced support and clinical advice to care home staff to enable high quality standards of care to residents with a focus on prevention of avoidable hospital admissions.
The CHLN will work in partnership with the MDT team and Care Home to ensure safe and effective care using a quality improvement approach to improving outcomes for residents. The post holder will hold an adult nursing registration with considerable relevant experience and transferable skills and will have or be willing to undertake further professional development related to the role.
